ASSUMPTION OF RISK

I acknowledge that participation in boxing training and related physical activities involves inherent risks including but not limited to physical injury, muscle strain, joint injury, concussion, and in extreme cases permanent disability or death. I confirm that I am voluntarily participating in these activities with full knowledge of the risks involved.

MEDICAL FITNESS

I declare that I am in good physical health and know of no medical condition that would prevent my safe participation in boxing training. I agree to inform NXT Gen Boxing Club PTY LTD of any changes to my health that may affect my ability to train safely. I acknowledge that NXT Gen Boxing Club PTY LTD is not a medical provider and does not offer medical advice.

RELEASE OF LIABILITY

In consideration of being permitted to use the facilities and participate in training at NXT Gen Boxing Club PTY LTD, I hereby release, waive, and discharge NXT Gen Boxing Club PTY LTD, its owners, coaches, employees, and agents from any and all liability, claims, or causes of action arising from my participation, including those caused by negligence.

MEDIA CONSENT

I consent to NXT Gen Boxing Club PTY LTD photographing or filming me during training sessions and using such material for promotional purposes including social media, unless I notify the gym in writing that I do not consent.

MEMBERSHIP & PAYMENTS

I understand that my membership fees are billed weekly and that I am responsible for ensuring my payment details remain current. I understand that failure to maintain payment may result in suspension of my membership.

By signing up I confirm that I have read, understood, and agree to the terms above. I am 18 years of age or older, or have obtained parental/guardian consent.
